Job Title: Housing Led Support Service Case Manager
Location: Mobile, home working and hot desking where applicable
Salary and Benefits: £33,250 FTE pro rata, competitive salary, pension, health and wellbeing scheme
Contract Type: 30 hours per week

St Paul’s vision is that everyone has a place where they belong, so that homelessness is rare, brief and seldom reoccurring. At St Paul’s we use trauma as a lens through which we look to understand. In our experience, trauma, often from early life relationships and frequently reoccurring, is prevalent in many of the people who access our services. In supporting people to recover, we have learnt that consistency and trust is the essential ingredient.
The Role:
· Prevent and reduce repeat homelessness occurring within the lives of individuals experiencing multiple disadvantages.
· Deliver support to help sustain accommodation, improve stability and address mental ill-health, substance misuse, offending histories and social exclusion.
· Promote choice and control, build on strengths and recognise aspirations. Offer opportunities to recover and re-connect.
· Establish positive relationships with a wide range of voluntary and statutory services to improve external support available to clients.
Key Requirements:
· Experience of working with adults with relational or compound trauma, substance misuse, mental health, neurodiversity and co-occurring disorders.
· Good knowledge of Adult Safeguarding Policies, Procedures, and risk management.
· Capacity to work reflectively and be aware of one’s responses to others.
· Experience of negotiating and coordinating services that meet the needs of the individual
Key Responsibilities:
· Using a trauma informed approach, address the root cause of homelessness and support the individual to develop tools needed for long-term self-sufficiency.
· Actively deliver support to clients in their own accommodation to establish and sustain tenancy.
· Manage a caseload and act as the lead professional on cases when adopting a multi-agency approach.
· Identify and navigate resources to overcome barriers and create pathways into accommodation and services to achieve long-term stability
