Job Title: Transitional / Resettlement Worker
Location: Office-based with flexibility
Salary and Benefits: Salary £33,000 – £34,200 per annum, competitive salary, pension, health and wellbeing scheme

• Residents feel safe and secure following move on from the Hostel
• Effective multi agency working to ensure a holistic approach to an individual’s move on
plan
• Educating partners in the support needs of St Pauls residents and the homeless cohort to
Key tasks
• Provide proactive, person-centred support to clients who have recently moved on from
the hostel into the community
• Work closely with the Move-On Worker to ensure continuity of support and a smooth
transition into independent living
• Build strong, trusting relationships with clients to encourage early communication of
any concerns or challenges
• Conduct regular home visits to anticipate, identify, and resolve emerging issues early,
preventing risks to tenancy sustainment, wellbeing, and safety
• Support clients to understand and manage their tenancy responsibilities, intervening
early where difficulties arise
• Assist with budgeting, managing bills, and maintaining financial stability to prevent
arrears or debt
• Help clients set up and sustain utilities, benefits, and essential services, ensuring
stability from the outset
• Identify early warning signs of tenancy breakdown or disengagement and implement
timely, preventative interventions
• Support clients to access healthcare, mental health services, substance misuse
support, employment, and education, reducing risk factors linked to homelessness
• Encourage positive community integration and reduce social isolation to strengthen
long-term stability
• Work collaboratively with landlords, housing providers, and partner agencies to
resolve issues before they escalate
• Maintain accurate, timely records of support provided, risks identified, and
preventative actions taken
• Safeguard clients by recognising and responding to risks in line with organisational
policies

Transport requirements- Use of car for work purposes
Working pattern – Occasional Saturday or Sunday working.
Working conditions – Potential for hybrid working
Key internal relationships – LMT, Project Worker Team
Key external relationships – Local Authority Housing Teams, H&W Mental Health Trust,
Probation Service, Maggs, External Support Services (Statutory
and Voluntary Sector), PRS, RP’s,
Monthly Report to Housing Manager and CEO:
– Overall profile of residents – length of time in tenancy, accommodation and support
needs and access ( to advise relevant districts of property demand etc)
– Significant updates on case-by-case basis
– Risks, successes, failures and learnings
