St Paul’s vision is that everyone has a place where they belong, so that homelessness is rare, brief and seldom reoccurring. At St Paul’s we use trauma as a lens through which we look to understand. In our experience, trauma, often from early life relationships and frequently reoccurring, is prevalent in many of the people who access our services. In supporting people to recover, we have learnt that consistency and trust is the essential ingredient.
The Role:
· Support Case Managers to deliver a high-quality, Housing Led service that improves health and wellbeing, sustains tenancies and reduces repeat homelessness through trauma-informed, strengths-based and multi-agency practice.
· Manage the service and team, providing oversight of complex cases, risk, escalations and decision-making.
· Promote the service to key partners and stakeholders to strengthen understanding, referrals and collaboration.
· Lead on service monitoring and reporting, including completion of quarterly reports, case studies and forward plans.
Key Requirements:
· Degree level education in Social Work, Health and Social Care or 6 or more years’ experience in similar field.
· Knowledge of Trauma Informed Care and Psychologically Informed Environments and evidence of practice in a relevant service setting.
· Significant experience of managing a team who are working with people who have experienced trauma or poor attachment that impedes their ability to form trusting relationships and/or are living with other co-occurring conditions.
· Insight into contiguous services such as Police, Housing, Social, Medical, Substance Misuse and Mental Health.
Key Responsibilities:
· Lead on safeguarding concerns and Complex Adult Risk Management (CARM) processes, ensuring appropriate escalation and multi-agency responses.
· Contribute to the development and maintenance of clear pathways into services, removing barriers to access and engagement.
· Ensure service delivery remains compliant with contractual, organisational and governance requirements.
· Attend and contribute to partnership and strategic meetings, presenting service activity, learning and impact where required.
